I UNDERSTAND AND CONSENT TO THE FOLLOWING TERMS:

I agree that all information and data displayed by or stored on information systems provided by the Foundation is proprietary to the organization.

I agree to adhere to the laws, regulations, policies, and safety procedures set forth by the Foundation, its subsidiaries, and affiliates, and directives issued by authorized personnel (O5 Order 109 § 1.1.285). I agree to adhere to local, federal, and state law except when superseded by the aforementioned laws, regulations, policies, and safety procedures.

I agree that all information and data displayed by or stored on information systems provided by the Foundation is collected, monitored, and may be used for investigative purposes. I have no expectation of privacy when entering, using, or storing information on this system (Foundation Internal Information Control Policy 1A § 2.1.4).

I understand that the information and data that may be acquired through viewing, researching, or interacting with information systems is classified and restricted to authorized individuals only (Foundation Internal Information Control Policy 1A § 5.1).

I accept the consequences of unauthorized use of information system resources or any attempt to block, bypass, alter, or otherwise interfere with security features or protocols in any information system resource, hardware, or software provided by the Foundation (Foundation Internal Information Control Policy 1A § 7.1).

I agree to hold harmless the Foundation and its agents or assigns for any injury or loss of life I may sustain or property damaged may be caused by my own actions or the actions of third parties during my use of information system resources, hardware, or software provided by the Foundation.

I authorize the Foundation and its assigns to perform any surgical or nonsurgical procedure, including invasive surgery, should such procedures be necessary to prevent imminent death or grievous harm to my person (O5 Order 109 § 1.1.285).

I understand that the viewing, research, and/or interaction with information systems provided by the Foundation may result in immediate death, paralysis, permanent insanity, or other mental or physical trauma to me and that the risks of such outcomes are beyond the control of the Foundation (Foundation Internal Information Control Policy 1A § 2.1.5).

I understand that unauthorized access, distribution, or acquisition of this information or any part of this information is cause for disciplinary action or revocation of Foundation employment or contract (Foundation Internal Information Control Policy 1A § 5.3). I understand that said disciplinary action includes, but is not limited to punitive measures including demotion, revocation of access rights, mnestic cauterization, temporary L-class epistemic confinement, termination, selective ego-narrative rescripting, and/or ontological deletion. (Foundation Internal Security Manual § 1.3.4).

I agree to adhere to Foundation identification protocols and properly identify myself when necessary (ID Protocol 3.4A). Should I fail to do this, I understand and accept the consequences of apprehension and disarmament by local security personnel followed by disciplinary action up to and including ontological deletion (ID Protocol 3.4B).

I agree that impersonation of authorized personnel (whether said personnel hold position or not at the time of said impersonation), including but not limited to Foundation staff members, security, military, medical, and scientific teams, D-class, engineers, and any other Foundation or non-Foundation affiliated person is cause for termination of employment, and may result in revocation of access rights, disciplinary action up to and including ontological deletion (Foundation Internal Security Policy 1B § 2.2.2).
